Nutrition Assessment
This assessment and nutrition coaching session will be customized to suit your personal history and nutrition goals. You will leave this session with specific and realistic strategies that will help you improve your diet, lifestyle, and health.
NUTRITION ASSESSMENT FEE: $145
55 minutes
Includes:
Comprehensive health, medical, lifestyle, and nutrition history review
Optional review of bloodwork results and current supplements
You will leave with a customized nutrition prescription that includes (as appropriate):
Nutrition education
Supplement recommendations
Lifestyle recommendations
Physical activity recommendations
Specific tasks to focus on until the next session to help you achieve your goals

Billing Information
For one-time payments, options include e-transfer or credit card. Packages that are billed monthly require that a credit card be linked to your file.
Your employer's health insurance plan or Employee Assistance Program may provide coverage for a Registered Dietitian or Nutritionist. A receipt will be provided for all services that you may submit to your extended health insurance for reimbursement (if eligible). If your insurance company does not cover the services of a Registered Dietitian or Nutritionist, your receipts are eligible for a non-refundable government tax credit (please visit Canada Revenue Agency).
Direct billing is available for Blue Cross, BPA, Canada Life, ClaimSecure, Chambers of Commerce, Desjardins Insurance, Equitable Life, First Canadian, Industrial Alliance, GMS, Johnston Group, Maximum Benefit and Manulife. Some insurers, including SunLife, do not permit dietitians to bill directly, but your receipts can be submitted for reimbursement if your plan includes Registered Dietitian benefits.
